1: How do I communicate changes to Excel (e.g address, TIN, health plan participation)?

Providers shall notify the IPA in writing (preferably on office letterhead) along with any required supporting documentation (e.g. a TIN change requires a copy of the W-9 form) and an effective date of the change.

2: How do I change my password for my Provider Login?

We recommend you change your password at least every 6 months, or as needed to maintain member/patient security. Contact Provider Services at (408) 937-3612 for assistance in changing your password.

3: As a PCP, why must I submit encounter data?

Encounter data is needed for the following reasons:

Encounter data is required by all Health Plans

To provide the IPA with your comprehensive utilization reports

To capture data used for various financial incentive programs for you and Physicians Medical Group of San Jose.
